Revised RET-276 Now Available Online
A new revised version of the Separate Insurance Benefits Plan for Law Enforcement Officers / Beneficiary Designation Form, form Ret-276 (Rev. 5/04), is now available to human resources and benefits professionals on the Employer Links web page.
The form Ret-276 is completed if the law enforcement officer wishes to name someone other than his or her spouse as beneficiary under the Separate Insurance Benefits Plan. The employer completes the first part of the form Ret-276, which certifies the member's eligibility as a law enforcement officer, and the employer then gives the form to the member to designate his or her beneficiary under the Separate Insurance Benefits Plan.
The Employer Links web page includes other helpful tools for human resources and benefits professionals, including updated versions of employer manuals and Retirement Systems Law Books.
