Retirement System Division Forms (Employers' Page)

You can view the North Carolina Retirement Systems’ forms with Adobe Acrobat Reader. These forms have been saved in Adobe Portable Document Forms (PDF). In order to view the document, you must have Adobe Reader installed on your personal computer. There are directions to download a free Reader at the bottom of this page. Once you have the Reader, click on the form you wish to view.

Tip: Encourage the employee to download forms by logging into ORBIT, an acronym for Online Retirement Benefits through Integrated Technology.

Tip: Whenever possible, encourage the employee to type the answers to the form while working on the computer. The employee can keep the Guides; they are for the employee's information.
