Unclaimed Property Reporting Forms may be printed below using either Microsoft Word (Microsoft Excel for ASD-127 [Abandoned Tangible Personal Property]) or Adobe Reader 5.0 or above. In lieu of printing forms, we are requesting that holders submit their report via diskette in the specified NAUPA formatHRS Pro Software can be used to create electronic unclaimed property reports for submission to State Unclaimed Property Offices.  License for the single-user Standard Edition is free.  The license for the Enterprise Edition is available for a fee. Please submit a hard copy of your report along with the diskette for internal purposes.

NOTE: An ASD-159 or ASD-159G (Report of Unclaimed Property Verification and Checklist) must be completed, notarized, and submitted with any remittance (cash, securities, or tangible).
