Reporting Forms and Software
Unclaimed Property Reporting Forms may be printed below using either Microsoft Word (Microsoft Excel for ASD-127 [Abandoned Tangible Personal Property]) or Adobe Reader 5.0 or above. Once printed, using legal size 8.5 x 14 paper for all reporting forms except the ASD-127 which is standard 8.5 x 11, complete the form and mail it to the address listed on the form.
Beginning January 1, 2010 holders reporting 50 or more property owner records shall file the report in the NAUPA format. For holders reporting less than 50 property owner records, the report may be filed electronically or in paper form. To meet this requirement, holders may utilize HRS Pro which is a free electronic reporting software. Property owner information may be manually entered into this reporting software or may be imported into the software from other electronic applications such as excel.
For those holders reporting electronically, we are requesting that the ASD-159 (Unclaimed Property Verification and Checklist), or its equivalent, be submitted in paper format, along with a paper copy or PDF file of the owner detail to assist with the loading and balancing of holder reports.
Holders reporting tangible property should continue using paper reports ASD-127 (Abandoned Tangible Personal Property), regardless of the number of owners, unless advised otherwise.
NOTE: An ASD-159 or ASD-159G (Report of Unclaimed Property Verification and Checklist) must be completed, notarized, and submitted with any remittance (cash, securities, or tangible).

Holder Refund Request Form
Holders requesting a refund for funds submitted in error or refunded to owners should complete a Holder Refund Request Form. Once printed, using standard 8.5 x 11 paper, complete the form and mail it to the address listed on the form. Don't forget to include documentary evidence when submitting refund request.
